Bicycle Lock Cut Request Form

Transportation & Traffic Management can assist you if your bicycle lock is jammed, broken, or if you lost your key. Please complete the form below to submit a request for a bicycle lock cut and we will be back in touch with you to schedule an appointment.

Please note that we only cut locks on bicycles that are on The Ohio State University campus. Proof of ownership must be presented, a completed lock cut authorization form must signed, and an OSU ID or state issued Driver's License must be shown prior to this service being performed. In addition, please bring the bike key with you if available (i.e. not lost).
